Concern Home Care is proud to partner with New York State with its CONSUMER DIRECTED PERSONAL ASSISTANCE PROGRAM ... where through the Medicaid funded program our clients have the option of choosing a Family Member, Friend, Neighbor, Colleague, Parent, Spouse or Child take care of them through this familial act and program. The self-directing allowance to designate who will care for them, allows the client to choose a representative and patient advocate to assist in their care. 

*It is important to note that CONCERN HOME CARE, INC., is a Fiscal Intermediary for NYS CDPAP. It must be understood that we ourselves to not administer the actual CDPAP program, and all enrollment requisites and consumer's official application must be done so through self-directing or through a chosen representative of your choice. CONCERN HOME CARE, can help you  you navigate through the process, but ultimately the consumer and his or her family must ensure that they meet all New York State eligibility requirements (including being a Medicaid recipient)...and, in addition must be process through Local New York State Departments of Social Services.
